Collecting information from a large group of pepole can be a tricky process, involving designing a custom form in HTML, or cutting and pasting responses to a group electronic mail.
This updated version of Clipboard makes it not hard to design, administer and collate results of a simple questionnaire. You use a smiple form to specify question text, type (tickbox, text box etc) and response options, or use XML-like markup tags. Data is captured in tab-separated format, which you can export by electronic mail as an attachment, or cut-and-paste into Excel. You can also view topline results online.

* save and return later option for respondents to save their draft responses and bookmark them
* multiple or just a single question per screen * progress indicator and navigation buttons
* multiple templates, for different survey designs
* unique ID capture - send unique links to a list of people, and track who responds
